Benefits Corner this month features the steps needed to complete your health exam to avoid a premium increase. Also featured are details on our social media contest and the start of open enrollment next month.

It’s the Home Stretch

You have until Sept. 30 to complete your physical

Follow these steps to avoid the 2019 premium increase:

  1. Complete the Registration & Consent Form and Authorization of Use & Disclosure of Protected Health Information Form - Employees and their covered spouses each must complete these forms, which are available on the Employee Information Center, or EIC.
  2. Biometric Measures and Physical Confirmation - Complete an annual physical exam between Oct. 1, 2017 and Sept. 30, 2018, as well as a standard lab-test blood draw. Have the “Biometric Measures and PhysicalConfirmation” form completed and signed. If you do not have a doctor, visit www.myuhc.com and log into your account. Then, click on the “Find a Doctor”link. You may also call 1-877-571-9867 for assistance.
  3. Submit the Forms - Submit the completed forms by Oct. 15, 2018. Scan and email to saia@trihealth.com or securely fax to 1-513-487-4672.

Note that the doctor visit and blood/lab results must be completed by both the benefit eligible employee and eligible spouse and the forms submitted by the October deadline. Employees and spouses who choose not to participate must pay the surcharge outlined:

  • Employee Only: $135 a month
  • Employee Plus One Dependent: $135 a month
  • Family Coverage: $135 a month

All benefit-eligible employees hired on or before Dec. 31, 2017 and eligible spouses that will be covered on the Saia health plan who submit their blood/lab results will not be subject to the premium charge for 2019. This includes those employees with 10 or more years of service

Also, remember to enter our social media contest for a chance at winning some great prizes. Employees who post a picture to Facebook, Twitter or Instagram using hashtag #SaiaHealthy will be entered into a drawing to win one of three prizes including a fitness kit, fitness tracker or a day of paid time-off!

To be eligible to win:

  • Submit your “Saia Healthy Directions” forms by Oct. 15
  • Post a photo on Facebook, Twitter or Instagram showing how you stay healthy using hashtag #SaiaHealthy, tagging Saia in the post
  • Like or follow Saia Careers on Facebook, Twitter or Instagram

The contest will run until Sept. 30 and is open to full-time employees hired before Dec. 31, 2017 and who have submitted their forms. Winners will be announced the last week of October.

Open Enrollment Starts Next Month

As the end of the year is upon us, subsequently so is our annual benefits open enrollment. Starting Oct. 15, benefit-eligible employees may begin enrolling in the health plan offered by Saia. The online open enrollment process will be available 24-hours-a-day, seven-days-a-week. All full-time employees desiring benefits must enroll even if they receive medical, vision and prescription coverage at no cost or have previously enrolled either as a newly hired employee or during past enrollment periods.

Enrollment ends Nov. 9. Any elections made during the enrollment process will be effective Jan. 1, 2019 and are binding until the end of the calendar year unless there is a qualifying event.  Examples of qualifying events include a marriage, birth, divorce or loss of other medical coverage. Any qualifying event that impacts benefits must be reported to the Benefits Department within 31 days of the event occurring.

Over the next few weeks you will receive additional communications regarding open enrollment. Begin preparing for open enrollment by reviewing your current benefits including who you’ve selected as your life insurance and 401(k) beneficiaries.
