Reminder - The 2021 Wellness Program
Saia cares about your health, and wants to provide you with resources to help you live better, both inside and outside of work. That is why we are offering a voluntary wellness program for all benefit eligible employees and spouses administered by Wellworks For You. Participants who complete the wellness program requirements will avoid a wellness premium increase effective January 1, 2022. Please note, it is the participants’ responsibility to submit the physician results form as part of the wellness program to Wellworks For You by October 15, 2021.
Employees and Spouses
Benefit eligible employees and spouses will avoid a wellness premium increase effective January 1, 2022, for completing an annual physical with lab work between October 1, 2020 and September 30, 2021, submitting the physician results form to Wellworks For You by October 15, 2021.
Wellness Program
By participating in the Saia 2021 Wellness Program, you will take a few simple steps to better understand your health.
Log into your Wellness Portal
Your account credentials have already been generated for you. You do not need to create a new account.
Use the username and password formats below to log into the Wellness Portal.
- Go to www.wellworksforyoulogin.com
- Your account credentials are listed here:
- Employee: saia + Employee ID Example: saia12345 Password: wellness21
- Spouse: saia + Employee ID Example: saias12345 Password: wellness21 - You will be prompted to create a unique password and accept the terms of the consent form
Forgot Your Username or Password?
- Go to www.wellworksforyoulogin.com
- Select the "Forgot Username or Forgot Password" link
- Enter your email address to initiate the password reset process or retrieve your username
Submit Your Form
Submit your form by October 15, 2021 to Wellworks For You in one of the following ways:
- Scan and email: forms@wellworksforyou.com
- Upload via the Wellness Portal
- Upload to the mobile app: Take a photo of your form using your smartphone, and upload it to the Wellworks For You mobile app
